Snelle diagnose van tuberculose bij dromedariskameel (Camelus dromedarius) met behulp van op laterale flow-assay gebaseerde kit.

Een nauwkeurige vroege antemortem-diagnose van tuberculose bij dromedariskamelen is moeilijk vanwege het ontbreken van een betrouwbare diagnostische test. De huidige studie had tot doel een laterale flow-assay-gebaseerde kit (snelle assay-kit) te evalueren bij de diagnose van tuberculose die immuno-chromatografische detectie van antilichamen in serum, plasma of volbloed gebruikt.

In een dromedariskamelenkudde bestaande uit 337 dieren in Bikaner, Rajasthan, India, werden 50 volwassen zwakke kamelen (11 mannetjes en 39 vrouwtjes) getest door middel van een enkele intradermale tuberculinetest (SIDT) en een snelle testkit. In totaal werden 14 dieren (2 mannetjes, 12 vrouwtjes) positief bevonden bij snelle test. Bij SIDT vertoonden vier dieren een positieve reactie in het nekgebied en zeven dieren in de staartbasis. Een ander mannelijk dier werd SIDT-positief maar negatief bevonden bij een snelle test; het stierf na 12 maanden.

Negen snelle assay-positieve dieren stierven asymptomatisch in een periode van 1 tot 11 maanden, waarbij postmortale tuberculose-laesies werden onthuld die werden bevestigd door Ziehl-Neelsen-kleuring en histopathologie. Geen tuberculeuze laesie was duidelijk bij het dier dat alleen positief werd bevonden in SIDT. Resultaten van de huidige studie gaven aan dat serologische tests, zoals een snelle testkit, kunnen dienen als een betrouwbare test voor antemortem-diagnose van tuberculose bij dromedariskamelen.

Ontwikkeling van een RNA-gebaseerde kit voor het eenvoudig genereren van TCR-gemanipuleerde lymfocyten om de prestatie van de T-cel-assay te regelen.

Op cellen gebaseerde testen om antigeenspecifieke T-celresponsen te volgen, worden gekenmerkt door hun hoge complexiteit en moeten onder gecontroleerde omstandigheden worden uitgevoerd om meerdere mogelijke bronnen van assayvariatie te verminderen. Het ontbreken van standaardreagentia maakt het echter moeilijk om de resultaten die in één laboratorium in de loop van de tijd en tussen instellingen zijn gegenereerd, rechtstreeks te vergelijken. Daarom zijn door TCR ontworpen referentiemonsters (TERS) die een bepaald aantal antigeenspecifieke T-cellen bevatten en continu stabiele resultaten opleveren, dringend nodig.

We hebben met succes een eenvoudige en robuuste TERS-technologie ontwikkeld die een nuttig hulpmiddel vormt om dit probleem te verhelpen voor veelgebruikte T-cel-immuno-assays. Om gebruikers in staat te stellen grootschalige TERS te genereren, ter plaatse met behulp van de meest gebruikte elektroporatie (EP) -apparaten, werd een RNA-gebaseerde kitbenadering opgesteld, die stabiele TCR-mRNA en een geoptimaliseerd productieprotocol biedt. Ter voorbereiding op de release van deze immunocontrole-kit hebben we optimale EP-condities vastgesteld op zes apparaten en een uitgebreid RNA-stabiliteitsonderzoek gestart.

Verder hebben we de productie van TERS op locatie gecoördineerd met 4 deelnemers. Ten slotte werd een vaardigheidspanel georganiseerd om de productie van TERS zonder toezicht in verschillende laboratoria te testen met behulp van de kitbenadering. De verkregen resultaten tonen de haalbaarheid en robuustheid van de kitbenadering voor veelzijdige interne productie van cellulaire controlemonsters.

De last van de variabiliteit die wordt geïntroduceerd door de HEp-2-testkit en het CAD-systeem in ANA indirecte immunofluorescentietest.

Volgens de recente aanbevelingen van het American College of Rheumatology, ANA Task Force, moet de IIF-techniek worden beschouwd als de gouden standaard bij het testen van antinucleaire antilichamen (ANA’s). Om het gebrek aan standaardisatie te overwinnen, hebben biomedische industrieën verschillende computerondersteunde diagnosesystemen (CAD) ontwikkeld.

Snelle diagnose van tuberculose bij dromedariskameel (Camelus dromedarius) met behulp van op laterale flow-assay gebaseerde kit.

Tweehonderdeenenzestig opeenvolgende monsters met vermoedelijke auto-immuunziekten werden getest op ANA door middel van IIF op een routinematige HEp-2-testkit (Euroimmun AG). Toewijzing van het resultaat werd gemaakt als consensus voor positief / negatief werd bereikt door ten minste 2 van de 3 deskundige artsen. ANA-IIF werd ook uitgevoerd met behulp van 3 CAD-systemen: Zenit G-Sight (n = 84), Helios (n = 85) en NOVA View (n = 92); menselijke evaluatie werd herhaald op hetzelfde substraat van elk CAD-systeem (respectievelijk Immco, Aesku en Inova HEp-2-cellen).

Om de resultaten te anonimiseren, noemden we deze drie systemen willekeurig A, B en C. We voerden een statistische analyse uit waarbij verschillende maten van overeenstemming tussen de beoordelingen werden berekend, en we verbeterden ook de evaluatie door de Wilcoxon-test te gebruiken voor niet-parametrische gegevens. Overeenstemming tussen de menselijke metingen op routinematig HEp-2-testkit en menselijke metingen op CAD HEp-2-test was aanzienlijk voor A (k = 0,82) en B (k = 0,72), en bijna perfect voor C (k = 0,89).

Dergelijke metingen waren alleen statistisch verschillend in geval A. Bij het vergelijken van de metingen van experts met de metingen van CAD-systemen, wanneer de monsters werden voorbereid met CAD HEp-2-testkits, vonden we een bijna perfecte overeenkomst voor B en C (k = 0,86; k = 0,82) en substantiële overeenstemming voor A (k = 0,73). Nogmaals, menselijke en CAD-metingen waren alleen statistisch verschillend in A.Toen we de metingen van medische experts op een routinematige HEp-2-testkit vergeleken met de output van de CAD-systemen die werkten met hun eigen objectglaasjes, vonden we substantiële overeenstemming voor alle systemen (A: k = 0,62; B: k = 0,65; C: k = 0,71).
