Tuberculose snelle moleculaire assays, waaronder GeneXpert MTB / RIF® en Loopamp MTBC Detection Kit®, zijn zeer gevoelig en specifiek. Dergelijke prestaties vertalen zich niet automatisch in verbeterde ziektebestrijding en zijn sterk afhankelijk van het gebruik ervan, de lokale epidemiologie en de diagnostische algoritmen waarin ze worden geïmplementeerd. We evalueren de prestaties van beide assays en beoordelen hun impact op de melding van aanvullende gevallen wanneer deze worden geïmplementeerd binnen door de WHO aanbevolen diagnostische algoritmen voor tuberculose in Madagaskar. 55 achtenveertig vermoedelijke longtuberculosepatiënten werden prospectief gerekruteerd tussen november 2013 en december 2014 in Antananarivo, Madagaskar, een hoge Incidentie van tuberculose in sub-Sahara Afrikaanse stedelijke omgeving.

Beide moleculaire testen werden geëvalueerd als eerstelijns- of aanvullende testen na negatieve uitstrijkmicroscopie. Op basis van lokaal gedefinieerde prestatiekenmerken van de assay meten we de impact van beide assays en door de WHO aanbevolen diagnostische algoritmen op aanvullende meldingen van tuberculose-gevallen. Hoge gevoeligheid en specificiteit werd bevestigd voor zowel GeneXpert MTB / RIF® (86,6% (95% BI 81,1-90,7% ) en 97,4% (95% BI 94,9-98,8%)) en Loopamp MTBC Detection Kit® (84,6% (95% BI 78,9-89,0%) en 98,4% (95% BI 96,2-99,4%)). Implementatie van GeneXpert MTB / RIF® en Loopamp MTBC Detection Kit® verhoogde de gevoeligheid van diagnostische algoritmen voor tuberculose van 73,6% (95% BI 67,1-79,3%) tot 88,1% (95% BI 82,8-91,9%).

Deze toename was het grootst wanneer moleculaire testen werden gebruikt als aanvullende test na negatieve uitstrijkjesmicroscopie. Als aanvullende testen verbeterden GeneXpert MTB / RIF® en Loopamp MTBC Detection Kit® respectievelijk case-detectie met 23,8 en 21,2% (p <0,05). Inclusief GeneXpert MTB / RIF® of Loopamp MTBC Detection Kit® moleculaire assays voor TB-detectie op sputummonsters van vermoedelijke tbc-gevallen kunnen de melding van gevallen in tbc-diagnosecentra aanzienlijk verhogen. Het detectiepercentage van tbc-gevallen wordt verder verhoogd wanneer die tests worden gebruikt als tweedelijns vervolgonderzoek na negatieve microscopieresultaten van uitstrijkjes. Een landelijke schaalvergroting en digitale integratie van moleculair-gebaseerde tbc-diagnosetesten toont beloften voor tbc-bestrijding in Madagaskar.

Prestatie-evaluatie van een indoxylsulfaatassaykit “NIPRO”.

Achtergrond De relatie tussen nierziekte en cardiovasculaire ziekte (HVZ) staat momenteel bekend als cardiorenaal syndroom. Indoxylsulfaat (IS) is een van de uremische toxines die de progressie van het cardiorenaal syndroom versnellen. Dit rapport presenteert een nieuwe methode om IS op een eenvoudigere manier te meten. Methoden We hebben de analytische prestaties geëvalueerd van een IS Assay Kit “NIPRO” geladen op LABOSPECT 008.

De geëvalueerde analytische prestaties omvatten nauwkeurigheid, precisie, verdunningslineariteit, detectielimiet (LOD), kwantificatielimiet (LOQ), hersteltest, interferentietest en vergelijking met testen die zijn uitgevoerd door middel van hogedrukvloeistofchromatografie (HPLC). Resultaten Totale precisie toonde een variatiecoëfficiënt van <5,3% bij IS-concentraties van 3,57-277,73 μmol / L, en bevredigende resultaten werden waargenomen in de verdunningslineariteit, LOD, LOQ, herstel en interferentietests. De IS-testkit “NIPRO” vertoonde een hoge correlatie met de conventionele HPLC-methode (r = 0,993). Conclusies De IS-testkit “NIPRO” toonde bevredigende analytische prestaties, en dit suggereert dat het binnenkort een andere veelgebruikte methode zou kunnen worden om circulerende IS te meten.

In dit artikel wordt een fosfaatassaykit in één cel gerealiseerd voor de elektrochemische detectie van intracellulaire fosfaationen in afzonderlijke cellen. De componenten van de fosfaatassaykit, waaronder maltosefosforylase, maltose, mutarotase en glucoseoxidase, worden elektrochemisch in een levende cel geïnjecteerd via een capillair van nanometerformaat met de ringelektrode aan de punt.

Deze componenten reageren met fosfaationen in de cel om waterstofperoxide te genereren dat elektrochemisch wordt geoxideerd aan de ringelektrode voor de kwalificatie van intracellulaire fosfaationen. Een gemiddelde lading van 1,7 nA werd verzameld uit acht afzonderlijke cellen, wat duidt op een intracellulaire fosfaatconcentratie van 2,1 mM. De vestiging in de elektrochemische meting van fosfaationen biedt een speciale strategie om de fluctuatie van intracellulair fosfaat bij afzonderlijke cellen te volgen, wat significant is voor het toekomstig onderzoek van de fosfaatsignaaltransductieroute.

In klinische laboratoria is het een gangbare praktijk om tests te verifiëren voordat ze worden gerapporteerd, herhaalde tests. Ons doel was om de verschillen tussen de resultaten van herhalingen van de ethanolconcentratie in het bloed (BEC) te evalueren en te rapporteren over de rol van herhaalde tests om het rapporteren van onjuiste resultaten te voorkomen. Documentbeheersysteem van het ziekenhuis door de procentuele verandering tussen herhaalde BEC-testruns te berekenen.

Om de klinische relevantie te beoordelen, werd de bias tussen twee resultaten uit dezelfde steekproef vergeleken met behulp van de 1988 Clinical Laboratory Improvement Amendments ‘(CLIA) proficiency testing toelaatbare totale fout (TEa) limieten. Uit een totaal van 1.627 BEC-tests uitgevoerd tussen januari 2017 en In januari 2018 werd 70% (1.133) herhaaldelijk getest. Hiervan resulteerden 830 in BEC’s tussen 0-5 mmol / L, waarvan 237 (28,5%) boven de 25% aanvaardbare TEa.

Description: Description of target: The FAM FLICATM Capase 1 kit uses a target sequence (YVAD) sandwiched between a green fluorescent label, FAM, and FMK to make a quick and flexible method to analyze active capases in apoptotic cells. These kits measure apoptosis by detecting active caspases in whole, living cells. They don't work by using antibodies or an ELISA. Instead, their methodology is based a unique cell-permeable and non-cytotoxic reagent called Fluorochrome Inhibitor of Caspases.
